Inspection review of the high energy arc flash at Turkey Point Units 3 and 4 on March 18, 2017
The NRC has completed its reactive inspection review (ADAMS ML17132A258) of the high energy arc flash inside the reactor coil cabinet of the Unit 3 3A 4 kilovolt (kV) switchgear,
which occurred on March 18, 2017, at Turkey Point Units 3 and 4. See below for the inspection report link:

The NRC continues to review, and is doing longer term follow-up related to this event.

The Union of Concerned Scientists (UCS) posted a blog, written by David Lochbaum that provides UCS’s interesting perspective and sometimes sardonic take on this event and the NRC.

The NIOSH Musculoskeletal Disorders Research Program recently updated the NIOSH document Elements of Ergonomics Programs – a step-by-step guide (https://www.cdc.gov/niosh/topics/ergonomics/ergoprimer/default.html) to create, implement, and maintain an ergonomics program. The guide provides basic information useful for employers, workers and others in designing an effective ergonomics program to prevent work-related musculoskeletal disorders. Summary of NRC Information Notice 2016-13: Uranium Accumulation in Fuel Cycle Facility Ventilation and Scrubber Systems

DOE facilities may have production facilities similar to those addressed in this information notice.
The notice addresses the potential for uranium accumulation in off-gas ventilation and scrubber systems and some potential causal factors that could contribute to this type of event.
Abnormal amounts of material buildup occurred in the inlet transition region and association ductwork.
Criticality mass limits were exceeded, posing a potential risk.
The NRC identified several causal factors:
  • Procedures did not provide the necessary details, clear instructions and acceptance criteria
  • Configuration management was not effective
  • Incorrect engineering and scientific assumptions were not challenged
  • Decision making was not conservative, introducing potential risk
  • Nuclear safety culture – some employees were not aware of criticality mass limits, ventilation modifications and other requirements
